    Disability among refugees and asylum seekers in Italy. Results from a global health perspective

    Limited evidence exists on the proportion ofrefugees and asylum seekers with disabilities, and knowledges about this target population are lacking. The objectives of the present investigation were a) mapping reception centers for hosting refugees and asylum seekers with disabilities; b) estimating the percentage of people with disabilities and examining the risks for specific migration routes; and c) highlighting the needs of refugees and asylum seekers using a global health perspective. We used the Washington Group Short Set Enhanced for estimating prevalence of disability, while Community-Based Rehabilitation Indicators were used to investigate their global needs. A total of 483 refugees and asylum seekers were interviewed on the whole national territory. The percentage of people with disabilities was 23.81, with a high prevalence of mental health problems, namely anxiety (18.22%) and depression (9.94%). Migrants who travel across the central Mediterranean route showed a high risk for disability condition (OR 2.08), with higher prevalence of anxiety (OR 2.19), while people who travelled the Balkan route seem to be a higher risk for mobility limitations (OR 3.03). The availability for hosting migrants with disabilities in reception centers is limited: 2.03% of the total national availability, with different distribution among regions. Disparities for each component of community-based rehabilitation indicators were also found, namely in health, education, social, livelihood and empowerment. Differences in access to healthcare services among migrants were also observed, in particular Ukrainian group seems to be more likely to get medical assistance, probably due to specific norms and communication strategies adopted by EU and Italy. The “Ukrainian Model” for refugees could be used for the general migrant population, in order to guarantee protection and adequate reception in host countries

Current sources of quantum light states tailored to measuring, allow to monitor phenomena evolving on time scales of the order of the second. These are characteristic of product accumulation in chemical reactions of technologically interest, in particular those involving chiral compounds. Here we adopt a quantum multiparameter approach to investigate the dynamic process of sucrose acid hydrolysis as a test bed for such applications.     Risk analysis for the Ancona landslide—II: estimation of risk to buildings

    This paper illustrates the quantitative estimation of specific risk (i.e., the product of hazard and vulnerability) for 39 buildings located upon the Ancona landslide based on the characterization of landslide kinematics presented in a companion paper. Hazard is quantified based on intensity, intended as the damaging potential of the kinetic and/or geometric attributes of the landslide, and is expressed in terms of expected exceedance of preset cumulative displacement thresholds for a set of five reference time intervals, ranging from 1 to 100 years. The estimation of hazard relies sequentially on (1) Monte Carlo simulation of displacement series, with sampling distributions of average yearly displacement defined on the basis of the statistical processing of inclinometer and radar interferometer data; and (2) the subsequent spatialization of displacement using radial basis interpolation as described in the companion paper. The vulnerability of the set of buildings relies on a quantitative model in which vulnerability is a function of landslide intensity and the resilience of the buildings. Resilience is a function of a set of indicators including structural type, age, and foundation type and is temporally variable due to the progressive structural degradation. Hazard, vulnerability, and specific risk are estimated for the set of five aforementioned reference time intervals.     Dorsal Prefrontal Cortex Impairment in Methoxetamine-Induced Psychosis: an 18F-FDG PET/CT Case Study

Epub ahead of print 13 February 2019Novel psychoactive substances (NPSs) have currently become a major public health concern because of relatively easy accessibility to these compounds and difficulty in identifying them with routine laboratory techniques. Here, we report the 18 F-fluorodeoxyglucose positron emission tomography/computerized tomography ( 18 F-FDG PET/CT) case study of a 23-year-old man who developed a substance-induced psychotic disorder after having intravenously injected himself with an unspecified amount of methoxetamine (MXE), a ketamine derivative hallucinogen. From a clinical perspective, a blunted affective responsiveness with diminished social drive and sense of purpose, along with a profound detachment from the environment, was observed. Psychometric and neuropsychological assessments highlighted severe dissociative symptoms and lack of motivation, along with a mild impairment of verbal fluency, working memory, and attention. Patient’s 18 F-FDG PET/CT scans displayed a significant bilateral deficit of tracer uptake within the dorsolateral prefrontal cortex (DLPFC). DLPFC activity is critical to goal-oriented cognitive functions, including working memory and sustained attention. DLPFC is also involved in both the temporal integration across multiple sensory modes and in the volitional control of actions, leading to the possibility to construct logically coherent temporal configurations of thought, speech, and behavior.     A Psychometric Properties Evaluation of the Italian Version of the Geriatric Depression Scale

    Objective. The Geriatric Depression Scale (GDS) is an evaluation tool to diagnose older adult's depression. This questionnaire was defined by Yesavage and Brink in 1982; it was designed expressly for the older person and defines his/her degree of satisfaction, quality of life, and feelings. The objective of this study is to evaluate the psychometric properties of the Italian translation of the Geriatric Depression Scale (GDS-IT). Methods. The Italian version of the Geriatric Depression Scale was administered to 119 people (79 people with a depression diagnosis and 40 healthy ones). We examined the following psychometric characteristics: internal consistency reliability, test-retest reliability, concurrent validity, and construct validity (factor structure). Results. Cronbach's Alpha for the GDS-IT administered to the depressed sample was 0.84. Test-retest reliability was 0.91 and the concurrent validity was 0.83. The factorial analysis showed a structure of 5 factors, and the scale cut-off is between 10 and 11. Conclusion. The GDS-IT proved to be a reliable and valid questionnaire for the evaluation of depression in an Italian population. In the present study, the GDS-IT showed good psychometric properties. Health professionals now have an assessment tool for the evaluation of depression symptoms in the Italian population

    The wheelchair use confidence scale: italian translation, adaptation, and validation of the short form

    Objective: We developed an Italian version of the Wheelchair Use Confidence Scale for Manual Users- Short Form (WheelCon-M-I-short form) and examined its reliability and validity. Methods: The original scale was translated from English to Italian using the “Translation and Cultural Adaptation of Patient Reported Outcomes Measures–Principles of Good Practice” guidelines. The WheelCon-M-I-short form was administered to experienced manual wheelchair users who had a variety of diagnoses. Its internal consistency and test–retest reliability were examined. Its concurrent validity was evaluated using Pearson correlation coefficients with the Italian version of the Wheelchair Outcome Measure (WhOM-I) and the Italian version of the Barthel index (BI). Results: The WheelCon-M-I-short form was administered to 31 subjects. The mean ± SD of the WheelCon- M-I-short form score was 7.5±1.9. All WheelCon-M-I-short form items were either identical or similar in meaning to the WheelCon-M-short form items. Cronbach’s a for the WheelCon-M-I-short form was 0.95 (p<0.01), and the test–retest reliability (ICC) was 0.978 (p<0.01). The Pearson correlation coefficient of the WheelCon-M-I-short form scores with the WhOM-I scores was 0.7618 (p<0.01). The Pearson correl- ation coefficient of the WheelCon-M-I-short form scores with the Italian BI scores was 0.638 (p < 0.01). Conclusions: The WheelCon-M-I-short form was found to be reliable and a valid outcome measure for assessing manual wheelchair confidence in the Italian population

    Reliability and validity of the geriatric depression scale in Italian subjects with Parkinson's disease

    Introduction. The Geriatric Depression Scale (GDS) is commonly used to assess depressive symptoms, but its psychometric properties have never been examined in Italian people with Parkinson's disease (PD). The aim of this study was to study the reliability and validity of the Italian version of the GDS in a sample of PD patients. Methods. The GDS was administered to 74 patients with PD in order to study its internal consistency, test-retest reliability, construct, and discriminant validity. Results. The internal consistency of GDS was excellent (α = 0.903), as well as the test-retest reliability (ICC = 0.941 [95% CI: 0.886-0.970]). GDS showed a strong correlation with instruments related to the depression (ρ = 0.880) in PD (ρ = 0.712) and a weak correlation with generic measurement instruments (-0.320 < ρ <-0.217). An area under the curve of 0.892 (95% CI 0.809-0.975) indicated a moderate capability to discriminate depressed patients to nondepressed patient, with a cutoff value between 15 and 16 points that predicts depression (sensitivity = 87%; specificity = 82%). Conclusion. The GDS is a reliable and valid tool in a sample of Italian PD subjects; this scale can be used in clinical and research contexts

    Quality of life in Parkinson’s disease: Italian validation of the Parkinson’s Disease Questionnaire (PDQ-39-IT)

    Translation and cross-cultural adaptation of the 39-item Parkinson’s Disease Questionnaire (PDQ-39) to the Italian culture was performed by Oxford University Innovation in 2008, but this version has never been validated. Therefore, we performed the process of validation of the Italian version of the PDQ-39 (PDQ-39-IT) following the “Consensus-Based Standards for the Selection of Health Status Measurement Instruments” checklist. The translated PDQ-39-IT was tested with 104 patients diagnosed with Parkinson’s disease (PD) who were recruited between June and October 2017. The mean age of the participants was 65.7 ± 10.2 years, and the mean duration of symptoms was 7.4 ± 5.3 years. The internal consistency of the PDQ-39-IT was assessed by Cronbach’s alpha and ranged from 0.69 to 0.92. In an assessment of test-retest reliability in 35 of the 104 patients, the infraclass correlation coefficient (ICC) ranged from 0.85 to 0.96 for the various subitems of the PDQ-39-IT (all p < 0.01). Spearman’s rank correlation coefficient for the validity of the PDQ-39-IT and the Italian version of the 36-Item Short Form (SF-36) was − 0.50 (p < 0.01). The results show that the PDQ-39-IT is a reliable and valid tool to assess the impact of PD on functioning and well-being. Thus, the PDQ-39-IT can be used in clinical and research practice to assess this construct and to evaluate the overall effect of different treatments in Italian PD patients

    Effect of different lignocellulosic fibres on poly(Δ-caprolactone)-based composites for potential applications in orthotics

    This work compares the mechanical and thermal behaviour of fully biodegradable biocomposites based on polycaprolactone reinforced with three different natural fibres, namely hemp, sisal and coir, for potential applications in the field of orthoses. The same properties were further compared to those of two commercially available materials commonly used in the same prospective field. The results confirmed that the addition of natural fibres, irrespective of the origin of the fibres (leaf, bast or fruit) to a biodegradable matrix allows for significant improvement of the mechanical behaviour of the ensuing composites compared to traditional thermoplastic materials used in orthotics
